Supplemental Application Documents

All applicants must submit a supplemental application in addition to the DICAS application. The supplemental application should be submitted through the DICAS portal to the supplemental application section. Although not required, applicants applying to pathway #1 are encouraged to select their community site and preceptor prior to applying to the program. Applicants applying to pathway #2 are required to provide potential sites and preceptors for all rotations when applying.

Applicants who have prospective preceptors should submit preceptor qualification form(s) to the supplemental application section of DICAS.
